Mind & Body: Physical Health, Mental Wellbeing and Healthy Living

Growing older doesn't have to mean slowing down. With the right support, small things like a medication review, a bit of pain management or a regular check-up can make a real difference to how strong, capable and well you feel. We help you stay ahead of these, so you can keep doing what matters to you.

Why Health and Wellbeing Matters as We Age

Physical and mental health both shapes how independent and engaged you can stay in later life. Small, consistent habits, staying on top of medications, moving regularly, addressing pain early, tend to have an outsized impact on how well you feel day to day.

Being proactive rather than reactive gives you more choice in how your health is managed, and more energy for the things you actually want to do.

Common Challenges

A few issues tend to come up again as people navigate health in later life:

Too many medications, not enough oversight

Managing several prescriptions from different specialists can get complicated and increases the risk of interactions.

Chronic conditions that need ongoing management

Conditions like diabetes, heart disease or arthritis require consistent care, not just a one-off appointment.

Pain that gets accepted rather than managed

Persistent pain is sometimes treated as something to live with rather than something to address.

Mental wellbeing left unspoken

Grief, stress, memory worries or poor sleep often go unmentioned, even when they're affecting quality of life.

None of this means something has gone wrong. They're common, and the right support can make a meaningful difference.

How Planwise Can Help

We help you navigate the health and wellbeing decisions that come with later life, including:

  • Medication management and polypharmacy reviews
  • Pain management and nutrition
  • Preventive care, checks, immunisations and continence assessments
  • Chronic disease management, diabetes, heart disease, arthritis
  • Allied health referrals, podiatry, physiotherapy, occupational therapy, exercise physiology, speech pathology, dietetics
  • Wellbeing practices and exercise, yoga, pilates, breathwork, journalling, women's and men's health

Planwise coordinates and refers. We don't diagnose or treat. Any health concerns are directed to a qualified GP or allied health professional.

How Planwise Helps You Stay Well

We help connect the different parts of your health, so nothing falls through the cracks.

Managing Your Physical Health

If you're taking several medications, a Home Medicines Review, a Medicare-funded, pharmacist-led check conducted in your own home, can help simplify your regimen and make sure everything's working well together. We also help you stay on top of chronic disease management, preventive checks, immunisations and nutrition, keeping you feeling your best.

Allied Health Support

Podiatry, physiotherapy, occupational therapy, exercise physiology and speech pathology can all play a part in staying mobile and independent. We refer you to the right allied health professional for your needs. We don't provide treatment ourselves.

Movement and Staying Active

Exercise looks different at every age and ability level. We help you find activities that suit your current capability, whether that's strength and balance classes, yoga, pilates or a walking group, so movement stays part of your routine rather than something to work around.

Mental Wellbeing

Your emotional wellbeing matters just as much as your physical health. Whether it's working through stress, grief, memory concerns or sleep, we help you find the right support, whether that's a psychologist, a support group or simply someone to talk things through with.
